NOTE: your headshot image must be CROPPED TO A SQUARE. Please make sure it’s not cropped too close to your face.

Clinic/gym photos are not required but strongly encouraged.

If you get an error when you upload your headshot or clinic/gym images, please submit without and email them to markduralde@gmail.com.
